A. Frame: KNS-90/Polycarbonate Full Frame (Boys & Girls with different colours)
B. Lenses: CR-39(Plastic Lenses)
C. Frame Size(s):32/34/36/38/40/42/44/46.
D. Cleaning Cloth with good quality for cleaning the lens.
E. Hard case fixed with sticker on front side of the frame box as following

1. National Programme for control of Blindness & Visually impaired along with logo
2. District Name
3. Name of the School

5. Name of the Student.
6. Student ID

8. Sex

The Tender application forms will be issued from Dt: 30-12-2024 to Dt: 06-01-2025 at O/o District Programme Manager, DISTRICT BLINDNESS CONTROL SOCIETY (DBCS), NTR District, Room No 207, A Block, 2nd Floor, New Government General Hospital, Vijayawada, NTR District. 
Last date to submit filled in application is on Dt: 06.01.2025, by 4:00 PM.
Date of opening of Tenders Dt:07-01-2025, at 9:00 AM at Joint Collector Office, NTR District.

Cost of Tenders application is Rs.1000/- (One Thousand Rupees) each by DD, EMD amount will be Rs.7,000/- Seven Thousand Rupees Only (NON-REFUNDABLE) per Bidder to be paid through Demand Draft in Favour of District Blindness Control Society, NTR District.
